List credentialing workflow attachments with signed download URLs

GET 
/credentialing-workflows/:workflowId/attachments

Returns every attachment on the workflow with a V4-signed GCS download URL. Results are deduplicated. Individual signing failures are reported in errors[] without aborting the batch. Returns 503 when the kill switch is off.
