Update effective dates on a practitioner-group association

/practitioners/:certifyPractitionerId/groups/:certifyGroupId/effective-date

Updates the effective start date and/or termination date for a practitioner's association with a specific group, optionally cascading the date change to the practitioner's network participation records within that group. Use this when the dates on an existing practitioner-group association need to be corrected or extended; the certifyPractitionerId and certifyGroupId path parameters identify the association. The practitioner-group association must already exist, and if terminationDate is provided, it must be after effectiveDate. Cascading is not purely controlled by cascadeToNetworkParticipation: if networkId is supplied, the network-scoped cascade always happens regardless of the flag; without networkId, cascade only happens when cascadeToNetworkParticipation is true. Past dates are allowed for both effectiveDate and terminationDate.
