Update practitioner facility associations within a group

/practitioners/:id/facilities

Associates the practitioner with one or more facilities within a group, creating new associations or updating existing ones; this endpoint follows upsert semantics, so existing associations are updated in place rather than replaced wholesale. Use this when adding the practitioner to a facility or updating facility-specific data; the request body must include a groupId to scope the associations. {id} is the certifyPractitionerId, and the group identified by groupId in the request body must already be associated with the practitioner. Only the facilities present in the request are upserted — facilities already associated with the practitioner but omitted from the request are left unchanged, not removed.
