Add a hospital affiliation to a practitioner

/practitioners/:practitionerId/hospital-affiliations

Creates a new hospital affiliation entry on the practitioner record; the server assigns a stable UUID v4 as the affiliation identifier and returns the full affiliation in the response body. Use this when a practitioner gains a new hospital affiliation that needs to be recorded; no Location header is returned in the 201 response, so use the id field in the response body to reference the created affiliation. {practitionerId} is the certifyPractitionerId, and the hospital affiliations feature must be enabled for the tenant (403 returned if not). Only name and type are required; all other fields are optional, and endDate must be on or after startDate when both are supplied.
