Adds / Terms / Changes counts

GET 
/api/v1/atc/counts

Returns how many providers were added, changed, terminated or deleted for the caller's tenant in a date range, split by level (net-new, group, network, location), entity type (practitioner, facility, group) and source (roster, platform, credentialing, monitoring, script, unknown). Counts are on event time and are a lower bound (completeness = best_effort).
