Facility Credentialing Workflows Export
Data dictionary for the facility credentialing-workflows Excel export. Every column produced by the export, grouped by the Excel tab (section) it appears on. All unnested tabs carry workflow_id and entity_npi (the facility NPI) as join keys back to Main_Workflow_Data.

| workflow_latest_status_change_reason | STRING | Reason recorded for the workflow's most recent status change, if any. |
| credentialing_cycle | STRING | Credentialing cycle type (e.g. Initial, Annual/Recredentialing). |
| previous_credentialed_date | DATE | Date of the facility's prior credentialing cycle. |
| credentialing_due_date | DATE | Date by which credentialing/recredentialing is due. |
| file_type | STRING | Most recent file-type classification for the facility (e.g. Clean, Dirty); defaults to "Unassigned" if never set. |
| file_type_reason | STRING | Reason associated with the most recent file_type change; defaults to "Unassigned". |
| workflow_type | STRING | Always "FACILITY" for this export; distinguishes facility workflows from practitioner workflows. |
| application_received_date | DATE | Date the credentialing application was received (workflow creation date). |
| application_completion_date | DATE | Computed date the application was completed; resolves DATA_MISSING / file-incomplete outreach blockers first, falling back to workflow creation date; NULL while the workflow is still in DATA_MISSING status. Always NULL in Open Workflow report files (report_type=open), since the application cannot be complete while the workflow is still open. |
